i like to make a note here for XenServer 5.5 with update 2:

After doing:
mkinitrd --with-usb initrd-2.6.18-53.1.13.el5.xs4.1.0.254.273xen.img 2.6.18-53.1.13.el5.xs4.1.0.254.273xen

i also had to move the new initrd-2.6.18-53.1.13.el5.xs4.1.0.254.273xen.img  
to initrd-2.6-xen.img

this is because the boot proces uses the shortnamed initrd file.</description>
